The Diabetes Awareness Program is a community-based program targeting individuals with diabetes. Its overall mission is to reduce diabetes and its complications through education. DAPP’s mission is to empower individuals at-risk for developing diabetes or its complications to take control of their lives through prevention and education awareness, which includes knowledge of diabetes, physical activities, preventive behaviors and support groups. Staff works with local neighborhood groups, churches, schools, businesses, and other health care providers to deliver services.
