Contactless Permitting Process 

Effective May 16, 2022, the Consumer Health Division will use a contactless permitting process which allows all customers to apply on-line.  

  • Customers must upload supporting documents when applying.
  • Failure to provide all required documents will result in the delay of processing your request.

All fees must be paid on-line. Fees may be paid by:

  • Credit or debit card
  • We do not accept cash or American Express.
  • Fees are non-refundable and non-transferable.

Registered Food Service Manager Certification


A manager/supervisor, who has completed a food service manager certification course from a Texas Department of State Health Services accredited institution, must be on-site while the establishment is operating.  Operators without certified personnel on duty during operations may be issued Municipal Court citations for this violation.  For additional information, contact the Consumer Health Division between 8:00 am - 4:30 pm at (214) 670-8083.

To submit an application, the following are needed:

  • Course Provider Certificate
  • Current Government-issued ID
  • Name on the certificate must match government ID
  • Application Fee $63.00 per year  
  • Registration is valid for up to 5 years depending on the course provider certificate expiratoin date.

Note: The department may refuse to issue or renew a food service manager registration or may revoke a food service manager registration if the applicant or holder makes a false statement of material fact in the application for registration.
