Registered Food Service Manager Certification

A manager/supervisor, who has completed a food service manager certification course from a Texas Department of State Health Services accredited institution, must be on-site while the establishment is operating.  Operators without certified personnel on duty during operations may be issued Municipal Court citations for this violation.

For additional information, contact the Consumer Health Division between 8:00 am - 4:30 pm at (214) 670-8083 or visit the office at 7901 Goforth Rd, Dallas, Texas 75238. Customers must arrive in the lobby at least 40 minutes before closing to ensure service.

To submit an application, the following are needed:

  • Course Provider Certificate

  • Current Government-issued ID

  • Name on the certificate must match government ID

  • Application Fee $30 per year                                                       (Registration is valid for up to 5 years depending on the course provider certificate expiration date)


Note: The department may refuse to issue or renew a food service manager registration or may revoke a food service manager registration if the applicant or holder makes a false statement of material fact in the application for registration.
