CDBG Drivers of Poverty Provider Requirement Details

Requirements to become a CDBG Drivers of Poverty (DOP) Childcare Provider.

-Funding availability is limited-

  1. Be located within one of the targeted zip codes
  2. Currently registered with sam.gov
    1. Obtain a Unique Entity Identification (UEI)number
    2. Be active on sam.gov
  3. Be in good standing with the Office of Secretary of State (Texas)
  4. Have an active License to Operate with HHSC
  5. Have a W-9
  6. Have a current HHSC Monitoring Report
  7. Become in compliance with all insurance requirements and remain in compliance according to the requirements
  8. Submit a list of potential children that qualify for the program (see CDBG DOP Childcare Parent Enrollment Requirement Details), to estimate a budget

CDBG DOP Parent Enrollment Requirement Details

Requirements to become eligible for services under CDBG Drivers of Poverty (DOP) Childcare.

  1. Live in one of the targeted zip codes (provide proof of residency) and be a female head of household and/or fall within the Average Mean Family Income (AMFI), as defined by HUD
  2. Currently working
  3. Apply for Child Care Group
    1. Become active on CCMS waiting list
    2. Complete requirements to stay active on waiting list
  4. Complete an application for assistance, include household composition, and provide proof of income per the HUD AMFI Guidelines
  5. Proof of guardianship of enrolled child(children)
  6. Check in quarterly with City of Dallas Childcare Specialist to remain active in DOP Childcare program to ensure you are in compliance with eligibility
