Vital Statistics Order Online​

A qualified applicant with a valid primary identification may request a certified birth or death certificate online through our third party vendor, VitalChek.

Long Form vs Short Form: What's the difference?

Know Before You Order

• We can only issue long form birth and death certificates for events that occurred within the city limits of Dallas from April 1983 to present. See Important Notice below.
• If the birth or death occurred within the city limits of Dallas BEFORE April 1983- DO NOT USE VITALCHEK. You must place an order through Texas Vital Statistics
• Abstract (short form) birth records are available for all births that occurred in the State of Texas from 1926 to present. 
• In addition to our fees, VitalChek will require you to pay an additional $10 service charge per order.
• Regular Mail fee is $1 (expected delivery is within 5-7 business days).
• Expedited UPS shipping starts at $19.50 (expected delivery is within 1-2 business days).
• Expected delivery dates are tentative. It can take up to 48 hours for our office to receive the order from VitalChek.
