What is the Continuum of Care?

The Continuum of Care (CoC) is a regional system that coordinates homelessness services across multiple organizations and communities.

Rather than operating independently, partners work together to ensure individuals experiencing homelessness are connected to shelter, services. and housing through a shared, structured approach.

  • The governing collaborative of the Continuum of Care.

    The All Neighbors Coalition is the local planning and coordinating body for the Continuum of Care in Dallas and Collin Counties. It is supported and led by Housing Forward, the region’s CoC lead agency. The All Neighbors Coalition brings together more than 140 partner organizations to:

    • Coordinate services across the system
    • Manage the Coordinated Access System (CAS)
    • Align funding and resources
    • ​Track outcomes and system performance

  • Through the Continuum of Care:

    • Individuals enter the system through multiple pathways
    • Needs are assessed using a shared process
    • Services and housing are prioritized based on vulnerability

  • The City of Dallas participates in the Continuum of Care as coordinating partner and a key public funder supporting the regional homelessness response system.

    The City support the system by:

    • Aligning departments and local efforts
    • Providing funding to support housing and service delivery through the CoC
    • Supporting outreach and engagement
    • Providing public education and resources
    • Coordinating with service providers and regional partners

    The City does not operate the system alone – it works alongside Housing Forward and partner organizations to strengthen coordination, expand resources, and improve outcomes across the region.
