If you need to reset your password send an email to
Make sure Perform is selected in the top left corner.
Select Performance Evaluations, then Performance Evaluations List. This provides a listing of all performance evaluations assigned to you that aren't completed.
Search in the name field for the current year (i.e. 2018 / 2019).
Review the listing and ensure all of your direct reports have a plan listed here.
If you are missing an employee please send an email to
with the employee's name and ID number. City University will help you ensure the employee is assigned correctly. Please allow 24-48 hours for a response.
