Dallas City Code, Chapter 12A
Requires city officials and employees to disclose certain conflicts of interest and recuse themselves from any discussion on the matter. Dallas City Code, Chapter 12A, Section 3(c)(2) stipulates the completed statement must be filed with the City Secretary.

Local Government Code 171
Requires City officials to disclose certain relationships with vendors. Local Government Code 171 Section 171.004(b) stipulates the completed forms must be filed with the official records administrator (Dallas City Secretary's Office) of the governmental entity. For copies of forms submitted by City Officials, please submit an open records request.

Form for City Officials (Affidavit of Abstention from Voting)

Local Government Code 176
Requires City officials and vendors disclose certain relationships. Local Government Code 176 Section 176.006(a)(1) stipulates the completed form must be filed with the official records administrator (Dallas City Secretary's Office) not later than the seventh (7th) business day after the date the person becomes aware of the facts that require the statement or questionnaire be filed.
